Blend Sign follow up is asking borrower to upload a document occasionally.

Has anyone else seen this? A follow up is sent to a borrower that requires a signature. The email that the borrower receives is asking them to upload a document, not sign the document. It's always the same follow up letter.

Lately this has been happening occasionally to us, but only when the follow up is requested through our LOS via the KM plugin. Just curious if anyone has heard of this before.
